We are aware that mental health
problems are often accompanied by other needs involving both the individual
and family members. This program is designed to facilitate access
to the full spectrum of hundreds of community-based health and supportive
services and to assist with housing in our short-term and permanent facilities.
Staff members, called case managers, serve as advocates. They help identify
needs and link the person and family members to resources within the
community.
Case managers also monitor progress and linkage to obtaining these services
and assist with necessary changes in these resources as an individual’s
needs evolve over time.
Some people face major barriers to finding and obtaining services. Case
managers can help open doors to meet many needs.
Additionally, ACT manages two transitional or short-term housing centers
in Tulsa. Support services are provided to help residents with the basics
of daily living challenges such as isolation, loneliness, budgeting and
finances, shopping. Additionally, we help support residents' efforts
in the areas of employment, training, volunteering and recreational activities.
Supervised living in our apartments provides a bridge to community living
for people with a history of serious mental difficulties who are stabilized
enough to live independently, supported by the guidance of staff members
and aftercare programs.
